c. Group or Individual ID number
Have your doctor's prescription available. This will include the name of your referring physician and their phone number. If you have not seen a doctor yet, feel free to call us and we will assist you in this process.
It's that simple! If you already have completed the above steps, and have a scheduled appointment here's the next step. You can download the initial paperwork to save you time when you come to see us. This paperwork must be filled out completely prior to your first visit.
